HIV Outreach Service Linkage Worker – Clinic based

Description: Retention-in-care-focused services for persons living with HIV requiring patient follow-up for missed appointments or provider-canceled appointments which are not rescheduled. 50% field work; coordinated training with Outreach programming. Motivational Interviewing and ARTAS evidence-based practice used to assess client barriers to accessing care and providing advocacy on behalf of clients to decrease service gaps and remove barriers to medical care helping patients to develop and utilize independent living skills and strategies.

Requirements: Bachelor’s degree or higher from an accredited college or university with a major in social or behavioral sciences or equivalent of documented paid work experience in providing client services to Persons Living with HIV/AIDS. One year paid experience required. Bilingual Spanish/English a plus.
Full Time 40 hours per week.
